In a trailer for the documentary, the Give Your Heart A Break hitmaker said: "I was left with brain damage.
"I still deal with the effects of that today. I don't drive a car, because I have blind spots on my vision.
"For a long time, I had a really hard time reading, reading was a big deal when I was able to read out of a book, which was two months later because my vision was so blurry.
"I dealt with a lot of the repercussions and I feel like they are still there to remind me of what could happen if I ever get into a dark place again."
Demi sparked concerns amongst fans when she released the single Sober just three days before her overdose, which heard her admitting she had relapsed after six years.
It was then revealed later that week that she had been blue lighted to Cedars-Sinai Hospital in Los Angeles.

Discussing why she has decided to open up about the ordeal, Demi continued: "I had so much to say over the past two years, wanting to set the record straight about what it was that happened. Any time that you suppress a part of yourself, it's gonna overflow.
"I had crossed a line that I had never crossed before… I snapped. I had three strokes.
"I had a heart attack, my doctors said that I had five to 10 more minutes [to live]," she continues to say in the trailer.
